Its called a thermal break. It helps keep the warmth inside the house and not
get transmitted to the ground outside. Not unusual for the top to get broken
off when the gravel base under the concrete gets spread. If you dug down, you'd
probably find it. In any case, missing a small piece isn't serious and its not
easily repairable.

Again, settling during construction isn't unusual and that's what the seams are
for. Unless the slab is cracked (and not just spider cracks), everything is
fine.
